I've tried unplugging, cleared EPG in menu and refreshed
Thanks for any help. It's been driving me crazy

EPG has nothing to do with it, try clear the cache, uninstall-reinstall mol2, maybe factory reset? If that doesn't help probably provider related.

I've had this quite a few times over the years. I do believe it is a provider issue. Do some movies/TV shows have icons? If some do, then probably not too much can be done.
Maybe try the connection on a phone or other device and see if there's icon there?

All channels have icons, it’s your box/connection that is fu*** up.
Earlier I had ADSL connection and in those days the box took long time to receive or update the icons.
Now I’m on fiber and after clearing everything upon a restart everything is there instantly, I have Z8 box by the way.
It can also be your provider that runs on too small servers with congestion, many things that can cause problems like yours.
You can always ask him to restart your connection, good luck 😉
