same here.... tried multiple servers.... it's either a problem with the stream over multiple provider (i have a residual service still in place) or it's a z10 issue. I notice it on multiple changes... even if I change servers and reset machine

1. Make short video from the issue with your phone and copy the video file to your pc.

Audiosync in most cases issue from provider or the connection with them. After Router and box reboot (on your side) or 1-2 days on provider side (server reboot) it works again fine. Audiosync was always temporarily issue.

Since switching from the Z Alpha to Z10 Max, noticed a lot of files in the VOD section with audio sync issues.
If I pause or rewind content briefly & unpause/play, then it seems back in sync for a few seconds, then you see a frame skip, then back out of sync again.
This seems to. be affecting a lot of files and did not have the same issue with Alpha with the same service?
I bought the Z10 about 5 weeks ago. I have the exact same thing/problem when watching VOD. It's indeed shortly in sync and after a short frame skip it's out of sync.
When watching TV i noticed, i have this only when the channel audio track is set to 5.1. When i set the audiotrack to stereo it's in sync. The source of the stream must be stereo (doesn't excist 5.1) and if set to 5.1 it needs to be converted.
My Z10 is connected with HDMI to a Denon receiver, same as some other devices. I never noticed a audio/video sync problem with any of these devices. Not with Prime or Netflix. Not with my Intel NUC HTPC with Kodi. So i do not think it can be a problem with my Receiver.
At this moment it looks like it occurs with more channel (more than 2) audio tracks? I will test some more by installing kodi or plex on the Z10 and connect to my Nas. I am sure that all the movies and series on the Nas are ok, since a have all seen them with the HTPC.
All tips i have done. Installed Audio Optimizer, run and reboot. One thing i would like to know. The setting buffer time, what does it excactly do? I read try to set it to 3 or 5 or 10....... What does it technicly do? I am not a fan of try and error... 
It preload AV stream into RAM (1-10sec.) before the stream begins to buffer, could also help in out of sync. If conn. is stable use lower buffer, if not higher.

My internet is glass fiber 200 mbit up/down. My audio Cabinet, has a switch, NETGEAR ProSAFE managed Plus Switch - GS108E. It supports IGMP Snooping. They say this helps with IPTV? My box is connected to this switch.
What player next to MyTVOnline, would be the best, to play and test files from NAS? I have only experience with Kodi op HTPC.

My tests from many providers tell`s only this: If there is some provider with constant audio sync issue, it will never work with them like it should work. One check with some trial account from other provider will show you there are differences. Audio sync and like you say picture skip could be some problem in routing, big latency, ping etc. there can be so many reasons for this. Server have maybe low resources or just to high load to deliver everything to all clients like it should, then such issues happen. of course there is also ISP that can block or throttle. etc. IPTV is very sensitive.
What can you do:
1. Restart your Router, Box and Switch. power off - power on. Btw. Switch with IGMP Snooping is ok and should not be the problem, but each device should from time to time be restarted.
2. Try other free trial 24h account from other provider.
3. Install vpn app to bypass the stream over vpn server. ( This can make it better or Worser, depends from routing and place of the issue).

Use Kodi again, is in my eyes the best and very powerful. VLC and MX Player are also good.
The skip seems to go away if you change the audio to PCM or stereo when watchin vod files , mine seems to stutter when playing aac5.1 audio
